How do I get only the vocals from a song?

How clean is the acapella, and what does it depend on?
On a studio recording with the voice up front, the vocal is usually good enough to remix. The singer's reverb and echo stay in it, and backing vocals usually stay with the lead. Dense choruses, live recordings and low-bitrate files are harder: faint traces of the instruments can remain.
Which files work, and what are the limits?
MP3, WAV, M4A, FLAC, OGG and most video files, up to 60 MB and 6 minutes. Without an account you get up to 3 tracks a day; a free sign-in gives you up to 40 an hour. There is no paid tier and no watermark.
